Online Course Request (Parent Form) for ACTC Dual Credit Courses

The Online Course Request (Parent Form) must be completed in order to take an ACTC dual credit course.  

1. Completing this form does not enroll you in the course - ACTC will enroll students after confirming with guidance counselors on courses students are approved to take.
2. ACTC cannot enroll you in a course until this form is completed.
3. A parent or guardian must sign this course request form.
4. Do not complete this on a phone - the electronic signature will not work.
5. You will need your KCTCS ID to complete the form. Retrieve your ID
