Job Description
The Community Health Worker will assist high-risk patients with chronic disease management and care coordination in their home and community. As a member of the care coordination team, they will interact with patients through weekly home visits, and facilitate communication between the patient and their provider. This is a full-time non-exempt position. Work will be mostly remote, with some meetings at the clinic office.

$15-$17/hour

Skills Required
Interpersonal communication skills; counseling and health promotion skills; excellent language skills, including reading, writing, and speaking; basic computer skills; extensive knowledge of community resources; cultural competence dealing with a diverse patient population; reliable transportation.
