FORMS
Forms must be submitted to AS_FIVCOSEF@kctcs.edu no later than January 18, 2019.
To be eligible for competition at ACTC FIVCO SEF, all exhibitors are required to adhere to the 2019 Intel ISEF Rules and Guidelines for scientific research protocol. The required forms and guidelines help students ensure that the research they are planning is safe, ethical, and approved by a parent, a teacher, and field experts. Students and mentors may use the Rules Wizard tool to help determine what forms are required for the project.
It is the responsibility of the student and the Adult Sponsor to evaluate the study to determine if the research will require forms and/or review and approval prior to experimentation, especially projects that include human participants, vertebrate animals, or potentially hazardous biological agents.
Failure to adhere to the Intel ISEF Rules and Guidelines may result in disqualification at any state of the competition, including revocation of awards and honors.
All participants must complete the following required forms and bring with them to place with their exhibit at the fair: Checklist for Adult Sponsor (1), Student Checklist with Research Plan (1A), and Approval Form (1B). HTML versions of all forms are available for easy completion.
All forms must be present at time of exhibit judging.
These forms need to be completed and present with each Science Fair project (team projects will submit Forms 1 and 1A collectively and Form 1B individually).

Any entry involving vertebrate animals, human subjects, recombinant DNA, or tissues must receive prior approval.
